Please complete the appropriate forms and bring them with you to your audition.
Forms will also be available at the audition, but completing the forms in advance will expedite the audition process.
The forms listed below are fillable PDFs which means they can be completed on your computer after they have been downloaded. It is recommended that you use Adobe Acrobat Reader (a free application) to complete the forms as the formatting requirements of the documents works best with that application.

If you are auditioning as a group (i.e., dance group, speciality act, etc.), each member of the group must complete the Photo Release form and the Rehearsal Performance Schedule Absence form.
