Follies Audition Forms
Please complete the appropriate forms and bring them with you to your audition. Forms can be either downloaded, filled and printed OR printed and completed by hand.
Forms will also be available at the audition, but completing the forms in advance will expedite the audition process.

Click on the blue “Download” button to download the forms. This will allow you to complete the forms on your device. You will then PRINT the forms to bring with you to the audition.

Click on the link to open the form in your browser so that you can PRINT the forms and complete by hand. Then bring the forms with you to the audition.
